[摘要]目的：改良目前治男性乳房发育症的手术方法，观察其效果。方法：手术前应用高频超声探头选择脂肪和乳腺组织均有增生的男性乳房发育症患4-18例，以腋皱襞前端为入路，单纯应用锐性吸脂针对增大的乳房进行肿胀抽吸治，吸出物送病理学检查。 结果：乳房胸部塑形良好，无血肿、乳头乳晕感觉障碍等并发症。胸部无手术痕迹残留。随访3～18月。病理检查结果显示吸出物中有增生的乳腺组织，病理特点符合男性乳腺发育症。结论：对于脂肪和乳腺组织均有增生的男性乳房发育症，锐性吸脂针腋皱襞入路单纯肿胀抽吸手术是一种可选的有效手术方法。

Tumescence tissue suction for gynecomastia with sharp cannula inserting through axillary furrow
Abstract：Objective To modify the surgical technique for the treatment of gynecomastia and verify the validity of themodification Methods 18 patients with gynecomastia were included in this study Both excess fat and parenchymalhypertrophy were detected in all breasts by high—frequency ultrasound．Tumescence tissue suction was solely applidto treat the gynecomastia The tip of suction cannula is sharp and the entrance of cannula is located at the axillaryfurrow The aspirates were examined histologically Results The chest contours were finely rebuilt and no complicationsoccurred such as haematoma，nipple sensory disabililty．What is important is there were no scars on chests and norecurrence in 3—18 months follow—up．Aspirates included mammary gland tissue and its histological characteristic wasin line with gynecomastia．Conclusion Tumescence tissue suction with sharp cannulas inserting through axillary furrowIs a valid surgical technique‘[or gynecomastia in which both excess ftat and parenchymal hypertrophy are existing．

男性乳房发育症是男性较常见的乳房疾病，约占男性乳房疾病的60％～80％，近年来该病的发病率和就诊率明显增加，而且多数患者均是因为局部形态异常及心理压力要求下手术治。因此，探索创伤更小、塑形效果更好的手术方法一直是整形美容外科医生的努力方向。我们应用锐性吸脂针以腋皱襞为入路治男性乳房发育症18例，效果良好，现报道如下。
1 临床资料
本组18例忠者，年龄18～34岁，平均26岁，体重67～92kg，平均83kg。均为双侧乳房肥人，尢严重皮肤松弛，无自觉症状。手术前进行高频探头超声诊断，显示脂肪组织和乳腺组织均有增生，乳腺呈轻中度发育，而且排除恶性病变的可能。18例患者均有1年以上的病程。
2手术方法本组手术均采用尖端形似注射器针头的锐性吸脂针，外擘直径2．5～3．5m，带有侧孔，侧孔的边缘为锐性，似刀刃。
于站位标记肥人乳房的范围，取平卧位，双上肢外展90。，常规消毒铺手术巾。2％利多卡因(含1／20万。肾上腺素)注射于腋皱襞前端皮内及皮下，11号尖刀片于腋皱襞前端切开皮肤皮下2mm，注水针由此进入，注射肿胀液(500ml复方乳酸钠注射液+400mg利多卡因+lmg肾上腺素)。肿胀液的注射量以达到手术区皮肤软组织发白发硬为准。锐性吸脂针，连接电动负压吸引器，吸力为80～100kPa。 先将吸脂针的开口朝下，回归抽吸乳房肥大区域的脂肪组织及乳腺组织，再将吸脂针的开口朝上，回归抽吸，至达到满意的厚度。乳头下方因有较多乳腺组织，抽吸时阻力较大，应在与周边相同厚度的层次下方回归抽吸，必要时捏起乳头及其下方组织，吸脂针在被捏起的组织之间抽吸，以利其问的组织被吸出。
直至乳房肥大区及周边皮下软组织厚度均一、平整，停止抽吸。缝合腋皱襞伤口，手术区敷垫棉垫，弹力绷带加压包扎。手术后24～48h换药。手术后7～lO天拆线．去除棉垫，弹力织物持续配戴4 周。
